Abstract: Introduction: The double burden of malnutrition (DBM) refers to the simultaneous manifestation of opposite forms of malnutrition, and can be considered an emerging public health problem. In contexts of social vulnerability, the challenges of malnutrition are amplified. DBM is related to multiple factors, including dietary patterns characterized by high consumption of ultra-processed foods. In childhood, DBM has been associated with several significant short- and long-term implications. Objective: To estimate the prevalence of double burden of malnutrition in children from Latin America and the Federal District, investigating its association with living in contexts of social vulnerability in the Federal District. Methodology: Two studies were conducted on the subject, using different methods: article 1 – Systematic review conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA)checklist, including observational studies that investigated the prevalence of DBM in children in Latin American countries. Indexed databases and gray literature were used,with no restrictions on language or time period. Three meta-analyses were calculated by grouping the studies into: coexistence of stunting and overweight; coexistence of anemia and overweight; and coexistence micronutrient deficiency and overweight. Possible sources of heterogeneity were explored through subgroup analysis, considering country,data collection location, and risk of bias; and article 2 - A cross-sectional study conducted with children in the 2nd to 4th grades of elementary school in public schools in the Federal District. A self-administered questionnaire was applied, weight and height were measured, and blood samples were collected. The analysis of the association between living in contexts of high social vulnerability and DCM, considering the simultaneous presence of iron deficiency and obesity in the same child, was performed using Poisson regression with robust variance, with a significance level of 5%. Results: Article 1 - The systematic review included 34 cross-sectional studies, of which 26 (76.47%) were classified as having a high risk of bias. The prevalence of coexistence of stunting and overweight, anemia and overweight, and micronutrient deficiency and overweight was4.00% (95% CI 3.00-6.00; I²=99.98), 2.00% (95% CI 2.00-3.00; I²=97.99%), and 8.00%(95% CI 6.00-10.00; I²=98.65%), respectively. Heterogeneity remained high even after subgroup analyses. Article 2 - 978 children were evaluated, with a mean age of 8.49 years(SD= 0.70). A subsample of 315 children underwent biochemical tests, in which serum ferritin and C-reactive protein (CRP) data were analyzed. The prevalence of DBM, considering the simultaneous presence of obesity and iron deficiency, was 4.85% (95%CI 2.15-10.58). The prevalence of obesity was 14.98% (95% CI 12.50-17.84) and iron deficiency was present in 8.93% (95% CI 4.72-16.25) of the children. There was no association between studying and residing in regions of high social vulnerability with a higher prevalence of DBM (p>0.05). Conclusions: The prevalence rates found highlight the need for greater efforts to address the double burden of malnutrition and that public food and nutrition policies need to consider the coexistence of forms of malnutrition, as well as the specificities of regions with greater social vulnerability.
