| Abstract: | Introduction: Kidney transplantation is the best treatment alternative for most patients with advanced chronic kidney disease. Despite the improvement in quality of life, kidney transplant recipients report low physical activity levels and a higher risk of developing cardiovascular disease compared to the general population, as well as functional impairments probably acquired in the pre-transplant period. High-intensity interval training (HIIT) may be a useful method for cardiovascular control, considering its impact on sympathovagal balance and functional performance. Objective: To investigate the effects of HIIT on heart rate variability and functional capacity in kidney transplant recipients. Methods: This is a quasi-experimental, descriptive, and quantitative study, with a pre- and post-intervention design. The training protocol was performed on a stationary bicycle, with three weekly sessions for 12 weeks. Before and after the training protocol, heart rate variability was collected at rest, in the supine position, using a Polar® cardiofrequency monitor and the functional performance test, the 30-second sit-to-stand test. Results: We performed 167 sessions, with an average of 24 sessions per volunteer, and we did not observe any adverse events during or after the HIIT sessions. Seven volunteers (3 men and 4 women) participated in the study, with an average age of 41±7.14 years, time since transplantation of 85±76.51 months, and the main cause of chronic kidney disease was arterial hypertension (71.4%). After HIIT, we observed an increase in parasympathetic modulation in time domain indices (rMSSD pre 26.56±22.98 post 27.99±17.70 p=0.01 and pNN50 pre 8.85±19.33 post 9.71±20.08 p=0.00) and the number of repetitions of the 30-second sit-to-stand test, but without statistical significance (pre 17.85±4.48 post 23.14±5.17 p=0.34). We found a positive correlation between functional performance and parasympathetic modulation (ρ = 0.52; p = 0.048). Conclusion: The HIIT protocol appears to positively influence parasympathetic modulation in kidney transplant recipients, promote better functional performance, and be a safe and viable option for exercise-based cardiovascular rehabilitation protocols. |
