Abstract:	Introduction: Adherence to the therapeutic plan after kidney transplantation remains a challenge for transplant recipients, their families, and healthcare professionals. Objective: To develop and analyze the evidence of validity of a health booklet for kidney transplant recipients. Methods: This was a mixed-methods study conducted in six stages: (1) scoping review; (2) focus groups with kidney transplant recipients; (3) investigation of the care context with healthcare professionals; (4) development of the health booklet; (5) evaluation by the target population; and (6) validation by expert judges. In the scoping review, data were analyzed descriptively. In the qualitative stages, textual data were processed using IRaMuTeQ® software through Similarity Analysis, Word Cloud, and Descending Hierarchical Classification. Evaluation by the target population employed the Agreement Index (AI), and expert validation used the Content Validity Index (CVI) and the binomial test, conducted in two rounds using the Delphi technique. The study was approved by a Research Ethics Committee. Results: The scoping review included 29 studies, which highlighted essential post–kidney transplant clinical care, such as infection and rejection prevention, medication adherence, clinical follow-up, nutritional support, physical activity, dental care, sun protection, and emotional support. The main technologies identified were educational, behavioral, and digital interventions. In subsequent stages, 24 kidney transplant recipients and 18 healthcare professionals participated, whose contributions resulted in two thematic axes: management of immunosuppressive therapy and dietary habits; and non-pharmacological care and health monitoring. Based on these findings, a 68-page health booklet was developed, predominantly green in color in reference to organ transplantation, and structured with clinical records and educational content to support post-transplant follow-up, including identification data, kidney function monitoring, laboratory tests, outpatient visits, hospitalizations, vaccination schedules, and self-care guidance. In the evaluation by the target population (n = 13), the booklet achieved a global Agreement Index of 99%. Expert validation, conducted in two rounds (24 judges in the first round and 19 in the second), demonstrated a global Content Validity Index of 98%, with most items achieving a CVI of 1.00 and statistical significance (p < 0.05). Conclusion: The study hypotheses were confirmed, resulting in the development of a validated educational and care technology grounded in scientific evidence and in the perceptions of kidney transplant recipients and healthcare professionals regarding post-transplant care. The booklet demonstrated content and face validity, methodological rigor, and alignment with post- transplant care needs, establishing itself as an effective tool to strengthen self-care and therapeutic adherence.
