Abstract: Cancer is one of the leading causes of death worldwide, accounting for approximately 9.6 million deaths annually. Treatment for this disease varies, with options ranging from individual to combined approaches, including surgical interventions, radiotherapy, and chemotherapy. Although current treatments are effective for certain types of cancer, they often have many side effects, such as nausea, dizziness, and vomiting, and can weaken the immune system by reducing essential leukocytes that protect against infections, such as neutrophils. As a result, cancer treatment has become a focus of extensive scientific research, leading to the search for more suitable therapies, including Photodynamic Therapy (PDT)—a safe and effective technique that is minimally invasive and less aggressive than existing therapies. PDT consists of three components: molecular oxygen, photosensitizing nanoparticles (PS), and light of a specific wavelength. This study emphasizes neutrophils, predominant cells in the innate immune response that play a role in the pathophysiology of various diseases, including cancer. We used label-free proteomics as an exploratory tool to identify deleterious effects on neutrophils, comparing control groups, neutrophils incubated with a phthalocyanine nanoemulsion, neutrophils exposed to light, and the complete PDT treatment group. We also performed more in-depth functional tests. Proteomics analysis identified a total of 4,678 proteins, considering an FDR < 1% and at least two peptides. Subsequently, a one-way ANOVA analysis was performed using the Progenesis software to identify ions, peptides, and proteins with significant differential abundance, resulting in the detection of 107 regulated proteins with at least two unique peptides. Proteomics analysis followed by functional tests revealed neutrophil activation after treatment, assessing neutrophil functionality in phagocytosis and migration, as well as the type of cell death—whether apoptosis, necrosis, or netosis. Thus, this study presents fundamental insights into neutrophil functionality following exposure to photodynamic therapy.
