| Abstract: | Introduction: Arly childhood is a crucial period for human growth and development, where adequate and healthy nutrition plays a fundamental role. Nutrition during the first thousand days of life, from conception to two years of age, is key to long-term health, influencing not only physical growth but also the cognitive and emotional development of children. Therefore, studying the dietary profile of children under two years old and the socioeconomic conditions in which they live is essential. It can support the understanding of risk and protective factors for health and contribute to the improvement of public policies aimed at child development. Objective: To assess the breastfeeding situation, food consumption, and food security of children aged 0 to 36 months assisted by the home visitation Criança Feliz Program investigating its association with socioeconomic indicators, social assistance benefits, and program adherence. Methods: Observational study with cross-sectional and longitudinal segments, conducted with children assisted by the Criança Feliz Program in the Federal District, Brazil. A structured questionnaire was applied to the caregivers to obtain information on socioeconomic and demographic aspects, breastfeeding status, food consumption, and household food security status. The dietary profile analysis was based on food consumption markers recommended by the Ministry of Health, and the Brazilian Food Insecurity Scale was used to evaluate household situations. Aspects of adherence to the Criança Feliz Program, participation in the Bolsa FamíliaProgram, and other social assistance benefits in the Federal District were also evaluated. Descriptive and multivariate statistical analyses were conducted, and Generalized Estimating Equations models with stratified analyses (p<0.05) were built. Results: The study included 301 children, 51.16% of whom were female. In 58.99% of households, the reference person was the mother: 86.20% were unemployed, and 27.08% had low educational attainment; 62.65% of beneficiaries lived on up to USD 200.00 per month. At the study's baseline, current breastfeeding status was associated with the child's age and maternal employment, being higher among mothers who were not working (p=0.015). The minimum dietary diversity among children older than six months was 62.21%, associated with adherence to the Criança Feliz Program (p=0.005). The consumption of ultra processed foods was 77.21%, associated with longer time in the program (p=0.047). The percentage of food insecurity was 83.9%, 65.3%, and 77.2% at baseline, after 6 months, and after 12 months of follow-up, respectively. There was a reduction in food insecurity after 6 months (OR: 0.79; 95% CI 0.70; 0.89) and 12 months (OR: 0.89; 95% CI 0.81; 0.97) of follow-up compared to baseline. In the stratified analyses, this reduction was observed among beneficiaries of social assistance programs in the Federal District and the Bolsa Família Program, among those who participated in the Criança Feliz Program for 9 months or more, and among families that received 4 program visits per month (p<0.05).Conclusions: The associations found in the study highlight the need to integrate food and nutrition education into initiatives targeting early childhood food consumption. Moreover, this study demonstrates that Criança Feliz Program participation and access to other social assistance benefits are essential for mitigating food insecurity among vulnerable children. |
