| Abstract: | The lingual frenulum, when presenting a morphology and insertions different from what is considered normal, can lead to limitation of tongue movements. Among the main hypotheses of repercussions caused by these limitations, changes in breastfeeding, speech and dental occlusion stand out. These repercussions and their relationship with the lingual frenulum are not yet well established in the literature. Thus, the objective of this study was to evaluate the influence of normal and altered lingual frenulum on breastfeeding time and on the development of speech and dental occlusion in children belonging to a birth cohort. A longitudinal study was conducted with babies born at the University Hospital of Brasília, participants in a birth cohort, who underwent the tongue test during neonatal screening and who returned for follow-up at 4 years of age. Data from the diagnosis of lingual frenulum at birth of children who were examined at 4 years of age were used. To evaluate breastfeeding and occlusion, all children who attended the consultation at 4 years of age were selected. Data on breastfeeding were collected from interviews and occlusion data were collected from clinical examination and application of Foster and Hamilton's criteria (1969). To collect data for speech assessment, a sub-sample of 126 children was selected and divided into 3 groups: G1- normal lingual frenulum; G2 – lingual frenulum altered without treatment; G3 – lingual frenulum altered with treatment and children performed naming and imitation tests (Wetzner, 2003, articulatory praxis (Hage, 2003) and assessment of anatomy and function of the lingual frenulum (Marchesan, 2010). The study was approved by the Committee of Research Ethics at the Faculty of Health of UnB (opinion: 2,052,955). The data were analyzed using the Stata 13.0 program, with a descriptive analysis performed with the presentation of relative and absolute frequencies and the Chisquare and Fisher's Exact statistical tests performed. and Poisson regression. Of 969 babies examined in the maternity ward, 277 returned at age 4. Of these, 58.3% breastfed exclusively for 6 months, at 12 months 69.2% of the children were still breastfeeding and at 24 months 45.4%. No statistically significant relationship was observed between the type of lingual frenulum stratified by whether or not a frenotomy was performed during exclusive (p=0.798 and p=0.492) or prolonged (p=0.172 and p=0.806) breastfeeding. Using a bottle and using a pacifier negatively influenced the total breastfeeding time. Regarding the present malocclusions, 6.0% of the children had an anterior crossbite, 10.9% an anterior open bite and 4.1% a posterior crossbite. No statistically significant relationship was observed between altered lingual frenulum and malocclusions based on the occlusion parameters evaluated. No statistically significant difference was observed in occlusion between children who underwent and those who did not undergo frenotomy and no statistically significant difference was observed in occlusion between children who underwent and those who did not undergo frenotomy. Regarding phonological processes, 53 children underwent assessments with speech therapists. A statistically significant relationship was observed between children who had altered lingual frenulum and underwent frenotomy with the presence of phonological and articulatory disorders when combined (p=0.03). A total of 33 (73.3%) children had an altered frenulum, according to the protocol used (Marchesan, 2010) and, despite there being a statistically significant relationship between a worse result in frenulum fixation and an altered lingual frenulum without treatment (p =0.023), between normal brake and the best result for clinical classification of the brake (p=0.006) and relationship between the diagnosis of normal and altered brake both by the Martinelli and Marchesan protocols (p=0.039), none of the children obtained total overall score of 25 points, a value that indicates interference of the lingual frenulum in the function of the tongue. It is concluded that the altered lingual frenulum was not a factor that interfered with the time of exclusive breastfeeding, the total time of prolonged breastfeeding, the development of speech and the development of occlusion. |
