| Abstract: | Individuals with chronic otitis media (COM) who have not yet had audibility restored are potentially subject to daily auditory difficulties, due to the perceptual barrier caused by tympanic perforation with or without otorrhea, leading to interruptions in the conversational flow. Goals: 1) To identify personal, otological and auditory characteristics, restriction to auditory participation (RAP) and use of communication facilitating strategies (CFE) among individuals with COM. 2) To investigate the judgment of communication partners about the RAP of the individual with COM, and the use of CFE by communication partners. 3) To evaluate the effect of psychoeducational rehabilitation groups on RAP and on the use of CFE by individuals with COM and their communication partners. Method. Fifty one adults with a history of COM (COM individuals), and 12 communication partners (companions) participated in this study. Personal, otological and audiological data were collected from COM individuals, and questionnaires were applied to verify level of RAP and use of CFE between COM individuals and companions. Sixteen COM individuals and four companions participated in a rehabilitation program, composed of five group meetings, in which a psychoeducational approach was carried out regarding communication situations and communication strategies. Restriction and strategies questionnaires were reapplied one and three months after rehabilitation, and results were compared to those of COM individuals who did not participate in the rehabilitation group. Main results: It was observed that: 41 COM individuals had had otological symptoms since childhood or adolescence, 20 had previously undergone surgery, 42 mentioned auditory difficulties in the interview, and 38 had signs of sensorineural alteration in the worst ear, with mean loss of 48 decibels. COM individuals presented variability in level of RAP and in use of CFE, but on average had moderate restriction and moderate use of strategies. RAP showed negative correlation with years of education, and higher RAP was associated with otological (suppurative COM) and audiological (mixed hearing loss, mild or moderate grade I) hearing loss in the best ear for participants with COM. RAP was influenced by years of education, and otological and audiological characteristics of the best ear among COM individuals. The effect of the rehabilitation program on COM participants was increase in the use of CFE, and awareness of negative feelings associated to hearing difficulties. Relatives of individuals with COM on both ears used more CFE than those with unilateral COM. The effect of the rehabilitation program on partners was to partially adjust the RAP judgment by two of them and to increase use of CFE only by one of them. Conclusion. Individuals with COM tend to have a long time of hearing deprivation, and a moderate level of restriction to auditory participation, despite individual variability. The rehabilitation program demonstrated potential to support individual with COM in dealing with their hearing difficulties, while audibility is not achieved by otologic surgery or other means. The participation of relatives in rehabilitation process of individuals with COM presented favorable potential for reducing conflicts in daily communication situations. |
